This application shall be considered active for no more than 30 days. After that time, applicants will be required to submit a new completed application.
In consideration for my employment, I agree to abide by the rules and regulations of the company,
which rules may be changed, Withdrawn, added or interpreted at any time, at the company’s sole
option and without prior notice to me.
I also acknowledge that my employment includes an introductory period. Completion of the
introductory period is not a guarantee that my employment will continue at Sheely’s. Employment is
“at will”, and may be terminated at any time, with or without cause, and with or without prior notice at
the option of the company or myself.
I certify that all the answers herein are true and complete to the best of my knowledge. I understand
that false or misleading information given in my application or interview process may be grounds for
refusing to hire me, or discharge should I be hired.
I authorize Sheely’s Furniture to make such investigation and inquiries of my previous employers,
education, criminal history, driving record, credit history, workers compensation claim history and
other related matters as may be necessary in arriving at an employment decision.
I hereby indemnify Sheely’s Furiture, and all their directors, officers, employees, and such agents
against any and all liabilities which may result from making such investigations or inquiries. I hereby
release employers, schools, or others persons or entities involved in releasing said information from
any and all liability related to the inquiries in connection with my application. Disclosure of the
requested information is voluntary.
I understand this application and authorization is not and will not be intended to be a contract of, or
offer of employment. I understand that the information may or may not be used in the final decision
necessary in arriving at an employment decision. It is my understanding that the information obtained
during this process is confidential and will remain in a designated flle. This information is not to be
released to other parties for any reason.
To the extent that a specific area of investigation is prohibited by State or Federal Law, that area will not be investigated. The other areas not prohibited by State or Federal Law will be investigated at the discretion ofthe company. A copy of this authorization may be accepted as an original.
